A PARTNERSHIP has been formed between five health bodies to take health care closer to people's communities.
Bury, Heywood and Middleton, North Manchester, Oldham and Rochdale primary care trusts, have joined together with the Pennine Acute Hospitals NHS Trust, tranforming care in the North-east of Manchester. The vision will focus on keeping people fit by offering high-quality services closer to people's homes in purpose-built health and social care centres.
The idea is to offer many services usually traditionally in hospitals on people's doorsteps.
