A BOLTON woman with a history of mental illness accidentally took an overdose of medication.
Jacqueline Chadwick, aged 45, was found in her home in Valley View, Bolton, last December 17, after Careline maintenance staff broke in.
Doctor Steven Wells, consultant pathologist said the cause of death was a combination of six different tablets and alcohol which together proved fatal.
The inquest heard that in the past Mrs Chadwick had struggled for many years with mental illness and did her best to get better.
However, when she felt increasingly unwell, she had a history of taking overdoses but she always called her family for help afterwards.
Coroner Jan Leeming recorded a verdict accidental death. She said: "I don't think Mrs Chadwick understood the combination of the tablets along with the alcohol, which formed part of her coping strategy, would have proved fatal and she would not have realised the consequences of her actions."
