A Bolton Coroners Court inquest heard how a man lay dying and ‘turned blue’ as a radiographer struggled to get help.

David Horsman suffered from multiple organ failure after having an anaphylactic shock during a regular CT scan at a mobile unit in the car park of the Royal Bolton Hospital as a result of a rare reaction to 'contrast dye', which is injected to help highlight areas of the body on the scan.

Radiographer Idongesit Okon asked for a hospital ‘crash team’ to be sent to the CT van in a call at 15.58, but the team did not arrive for 17 minutes after being sent to area E5, on the hospital's paediatric ward.

NWAS: Ambulance service, is the patient breathing?

IO:    Yes the patient is still breathing though the pulse is fine but we need you as fast as possible because it is a contrast reaction please

NWAS: It is an allergic reaction to contrast?

Male:    Yes please  (?? ) please

NWAS: OK, right tell me, is the patient awake at the moment?

Male:    The patient is not conscious because he is gradually getting worse

NWAS: Right, OK, is the breathing noisy?

Male:    The breathing is not noisy

NWAS: OK, right.  An allergic reaction to contrast

Male:    We need a rapid response please because he is in a very terrible state

NWAS: Yeah, ok.  I understand.  What is the address of the emergency?

Male: This is by Churchill Drive, A block, where we have the Covid Centre.  Where the Covid Centre is and where MRI scanner is.  Where we have the CT van, I’m at the CT van.

NWAS: So are you actually at the Royal Bolton Hospital?

Male: Royal Bolton Hospital

NWAS: OK, right.  And you say its Block A

NWAS Male: Hello, this is NWAS.  We have had a call from some gentleman about a patient who has had an allergic reaction to contrast.  Before I managed to get any information, he cleared on me. 

AP: I don’t know if it is a false call or he told me it was a cardiac arrest on ward E5.  I think he said A Block car park, he doesn’t understand much English. 

NWAS Male: All right, ok, so, can you just confirm because we are actually well, its actual hospital so if somebody had cardiac arrest, are crash team on the spot.

AP: If it is outside of the hospital

NWAS Male: So it is outside

AP: Yes, outside the hospital on the car park

NWAS Male: OK, car park at C block.

AP: A, A for Alpha

NWAS Male: A block, OK.  I can see that we have allocation, I am just going to put this in the note for the despatcher, could you just bear with me please?

AP: I will

NWAS Male: Right, so that’s A block.  Just confirm, it’s a cardiac arrest

AP: He told me, I was informed it was a cardiac arrest.

NWAS Male: Right

AP: Like I said he didn’t speak much English, he told me he was on ward E5, which apparently is on a car park

NWAS Male: Right OK

AP: So, it’s A Block car park

NWAS Male: A Block car park, ward E5

AP: Yes

NWAS Male: OK, thank you. Right would it be possible for you to try to connect me to him

AP: I don’t know who he was

NWAS Male: All right, if you can’t, that’s fine.  We already have a unit at the scene anyway, I mean at the hospital, its just the case of location but like I said, as far as can see we have one emergency ambulance at the scene anyway.  I am just waiting for the despatch to, you know, obviously to pass the message I have given them go to Block A car park, ward E5.
