A MOTHER who was murdered by her husband told friends, colleagues and doctors how he abused her in the months leading up to her death but a review has found her killing was not 'predictable or preventable'.

A fire started in the early hours of December 9 2013 at the home of Mahnaz Rafie, aged 48, and Hassan Raffie, aged 49, in Albert Road West, Heaton.

Their bodies were found in the kitchen and the body of Mrs Rafie's mother, Dolleh Joseph, aged 74, was found upstairs.

Mrs Rafie's cause of death was established as multiple stab wounds, while Mr Rafie and Mrs Joseph died of smoke inhalation.

A coroner recorded an open verdict on Mr Rafie and unlawful killing verdicts on Mrs Rafie and Mrs Joesph.

Greater Manchester Police recorded the incident as two murders, naming Mr Rafie as the person responsible.

The Be Safe Bolton Strategic Partnership has carried out a domestic homicide review into the death of Mrs Rafie.

DHRs try to ensure that public bodies, including social services, councils, police and other community-based organisations understand what happened that led to the death but they are not inquiries into how the victim died nor are they conducted to find out who is to blame.

It aims to find out whether there are any lessons to be learned and to make recommendations to prevent future homicides.

A report of the review does not identify the parties involved but looks at the murder of Mrs Rafie.

The review found that on several occasions Mrs Mohammadi gave information to her GP that should have been recognised as signs of domestic abuse and she was not questioned about.

Neither of the Bolton GP practices involved could show that a current domestic abuse or violence policy was in place.

In addition to this, Mrs Rafie told friends and colleagues about the domestic abuse she suffered but they did not know how to help her.

She told sales manager at the large national retail outlet she worked for that her husband 'controlled her life' and and that he had struck her.

A friend said that in September 2013 Mrs Rafie told her that her husband had hit her with a pan and grabbed her by the throat.

She told the same friend that she did not sleep well, believe that 'something would happen to her while she was sleeping'.

About a week before her death Mrs Rafie said she had woken up during the night with Mr Rafie standing over her.

She had also told the friend she was going to leave her husband.

Another friend told the review panel that Mrs Rafie had told her Mr Rafie had said: "If you leave me I will kill you and myself."

The review found that these friends and colleagues held facts which could identify domestic abuse but did not know how to help Mrs Rafie.

The review panel found that they did not view the death as being predictable or preventable.

It said that if the GP had made further inquiry into the couple's relationship, no agency could have reached the conclusion that Mr Rafie posed a risk of causing serious injury or death.

The review makes a number of recommendations, all of which have been accepted by the BSBS partnership.

These include developing a campaign aimed at raising awareness of domestic abuse and violence, to work with private sector employees to raise awareness about the need to recognise the indicators of domestic abuse and what to do if they suspect it is happening.

The panel also recommended developing targeted campaigns with the aim of raising awareness of violence against BME and marginalised communities and to continue to work with Bolton CCG to implement training for domestic violence awareness amongst all GPs in Bolton.
